To enroll in our course, please click on the document icon below to download our enrollment form. Adobe Acrobat Reader is required to open/view the application.  Once complete and signed, please fax or mail to us.

Tuition may be paid using any one of the following arrangements:

$445 to reserve your place in the course due one week prior to the start of class. The balance will be split into three payments. The first payment of $750 is due the first day of the course. The second payment of $750 is due by week 4 of the course. The remaining payment of $750 is due on or before week 8 of the course.

Students may also pay their deposit and then pay weekly installments via PayPal.  The subsequent weekly installments are a subscription.  Once the deposit is paid using "Add to Cart" use the "Subscribe" button to sign up, the account used will be charged $230/week for 10 weeks.
